Step 3: Sign and Submit to Fragomen

1. Sign and Date all Forms in the following fields requesting an Applicant signature, using a blue or black pen:  
   • U.S. Form I-485 – Sign and Date Page 15, Part 10, Questions 6.a. and 6.b.
   • U.S. Form I-131 – Sign and Date Page 5, Part 8, Questions 1.a. and 1.b.
   • U.S. Form I-765 – Sign and Date Page 5, Part 3, Questions 7.a. and 7.b.
   • Note: Parents can sign and date Forms for children under age 14 and write “Parent” in parenthesis next to the signature.
2. Sign and Complete the three (3) copies of U.S. Form G-28 for each applicant included in the AOS, using a blue or black pen. This Form is a notice to USCIS that Fragomen is counsel in this matter.
   • Print your Mailing Address on Page 2, Part 3, Questions 13.a. – 13.h. for each copy printed
   • Sign each copy in the signature box located on Page 3, Part 4, Questions 2.a. and 2.b.
3. Collect the required Supporting Documents for yourself and each family member: Please see the Form I-485 Supporting Document Checklist for instructions.
4. Provide six (6) passport-sized photographs for you and each family member applying with you. Write the name and alien registration number (if any) on the back of each photograph and place the photographs in a ziploc bag. See Important Notes for photograph requirements.
5. Follow the instructions provided to you by your Fragomen team to send the signed Forms and Documents to our office.
